I address my teachers as ‘Miss’ or ‘Sir’, and not because I don’t know their names, just because it sounds far too formal if I use their full name, or at least to me. Does this bother teachers?

I don’t think so. As long as you are not being rude, I would think teachers wouldn’t care what you call them.
